[DGD] Relocation of phantasmal

After the discovery of Github Pages, I have elected to relocate the
main phantasmal website from sourceforge.

Everyone please update your bookmarks and links:

Every url based on:

http://phantasmal.sourceforge.net/

Should be remapped to:

http://shentino.github.io/phantasmal/

The old site is no longer being maintained and will be removed on
March 24, 2018 as a tentative date.

Also, further development if any will take place on github.  The
sourceforge site is officially deprecated as of today.
