A bit of both.

I usually start with a basic idea of what I'd like to build. In order
to flesh that out, I need to test the idea by modeling it. In the
process of modeling it, I often need to ask who my target player is
before I can decide which of several approaches to take.

Once I have the model and I'm satisfied that it would work, I sit down
and write half the code, then become satisfied that the hard part is
done and move on to the next challenge. One of these days, I'll be in
the happy circumstance of having enough time, money, and manpower to
finish something. And when that happens, I'll pull out one of my many
half-finished projects and complete it.
