I know which zone you're talking about (the Caves outside of Qeynos) and
you're referring to Coldtooth on the 3rd floor. In fact, I was thinking
of that zone as I was reading your email even before I got to where you
mentioned EQ2. Coldtooth would sometimes come down to the 2nd floor and
if you were in the (curved, sloping) tunnel connecting the two, you have
nowhere to go and no time to react. A Scout with Tracking up would have
had some warning, but not anyone else. I've been tempted to take my high
level characters and just kill Coldtooth and the like over and over,
just for revenge. There's another super powerful mob in Oakmyst Forest.
Well...two of them but you have to go looking for one (Rotgut?) in a
cave and the other one comes looking for you.
In Anarchy Online, if you were Omni or Clan (sometimes even as a
Neutral) and took the wrong Grid exit, your char could be dead as soon
as it finished zoning.
