Exactly the same as you do on the web with a picture of such a
scene, or an X-rated story about it.  You don't link to it, and you
call the authorities.  They arrive at the hosting service with a
warrant and confiscate the disk drive.

If you run a hosting service, you have your customers sign a
contract concerning what type of content is and is not permitted on
your servers.  They violate it, you kick them off and they go
looking for someplace else.  Maybe they find adult-only servers that
are happy to take it.  For example, there are any number of Quake
III and Unreal Tournament servers running with nude player
models--they just don't advertise.

I think that this is not a new problem, and can be treated with the
same mechanisms already in place for other types of content.  The
*game* cannot enforce it, but it doesn't have to.
