Related to this is the difficulty reconciling PvE vs. PvP content.
In PvE players generally cooperate to consume content that shifts
based on their level.  In PvP, players are generally competing
against each other -- they ARE the content.

Reconciling these disparate play styles is extremely difficult
because they are fundamentally at odds with each other when it comes
to population distribution.  In a PvE game, you want to evenly
distribute the content as much as possible to avoid crowding and
competition for limited resources.  In a PvP game, you want to
encourage "crowding" to a large extent in order to foster
competition.
